Глагол - keeps, окончание -s - это показывает, что глагол стоит в Present Simple.
В Present Simple для вопросов есть два вспомогательных глагола DO или DOES. Для нашего предложения нам понадобится DOES (потому что у глагола окончание -s).
Строим общий вопрос: Does John keep fit? - теперь уже у глагола окончание -s не ставим (оно уже есть в DOES).
Специальный вопрос это вопрос с каким-нибудь вопросительным словом, например: Where does John keep fit? - посмотри, практически то же самое, что в общем вопросе, только на первое место поставили вопросительное слово.
Теперь вопрос к подлежащему - это вопрос КТО? WHO? Этот вопрос особенный - в нем нельзя ставить DOES: Who keeps fit? - окончание у глагола остается.
Разделительный вопрос (вопрос с хвостиком) строится так - переписываем предложение и приделываем вопросительный хвостик. Для этого нам опять понадобится DOES + местоимение, соответствующее подлежащему. Еще учитываем "правило батарейки" - если предложение утвердительное (не отрицательное), то хвостик будет отрицательный (с частицей not) и наоборот:

Альтернативный вопрос (вопрос выбора) всегда со словом или=or, начинается также как и общий вопрос: Does John or Mary keep fit?
